NDIS corrective action plan and non-conformity rectification

Strengthening NDIS compliance to support quality, accountability and continuous improvement

For profit provider

A registered NDIS provider that had recently completed a Stage 2 audit to expand its registration groups, including supported independent living (SIL), specialist disability accommodation (SDA) and high-intensity supports. The Stage 2 audit identified a number of non-conformances requiring structured remediation and validation against the NDIS Practice Standards. A large number of non-conformities with the NDIS Practice Stardards were reported.

HealthQ used our experience and understanding as NDIS consultants to identify the root cause for each non-conformance and developed a comprehensive action plan comprising 35 discrete actions.  We supported the completion of actions, attended the follow-up audit and were instrumental in registration being achieved.

  • Supporting evidence collation and alignment to audit requirements
  • Assisting with the progression and closure of identified non‑conformances with the external auditor
  • Embedding a clear, prioritised action plan to guide implementation

Process

The work followed a structured process that began with developing a clear and actionable roadmap to systematically address audit findings and strengthen compliance. This was supported by a detailed review and alignment of policies, procedures and service agreements to ensure they reflected NDIS requirements in a consistent and practical way. As implementation progressed, the organisation built confidence in managing audit obligations and engaging with auditors, supported by clearer evidence, improved documentation and a more coordinated approach. The process concluded with the establishment of a sustainable internal audit framework, embedding ongoing monitoring and continuous improvement as part of day to day practice

Outcome

This project enabled the NDIS provider to move beyond responding to audit findings toward building a proactive, internally driven compliance system. By focusing on both immediate actions and longer-term capability, the organisation was better positioned to maintain registration, manage future audits and deliver high-quality, compliant supports.
