Health Q Consulting (HealthQ) was engaged by Nunkuwarrin Yunti to provide support with the transition from the Australian Family Partnership Program (AFPP) toward a First Nations-led Maternal and Child Health Model of Care.

The program provides culturally informed, nurse-led home visiting support to Aboriginal and Torres Strait Islander mothers and families, with a strong focus on early intervention, prevention, and holistic care. The transition presented an opportunity to redesign services to better reflect community priorities, strengthen cultural governance, and improve integration within primary health care.

HealthQ was engaged to deliver a rapid evaluation of the existing program, to support a comprehensive understanding of the model’s performance and identify gaps and opportunities to support co-design and the development of a new, community-led model of care.

Activities Included

  • Development of a program logic model to define inputs, activities, outputs, and intended outcomes
  • Analysis of service data, performance trends, and client outcomes
  • Stakeholder consultation with staff, leadership, and program participants
  • Support for culturally appropriate consultation and community engagement processes
  • Facilitation of shared learning workshops to synthesise findings and identify future opportunities
  • Applicationof a mixed-methods evaluation framework integrating qualitative andquantitative insights

Process

The first component of the transition project was a rapid evaluation to support Nunkuwarrin Yunti’s transition to a First Nations-led Maternal and Child Health Model of Care.

We applied a structured, mixed-methods evaluation approach combining quantitative analysis with culturally informed engagement to generate insights grounded in both evidence and lived experience.

A key foundation of the evaluation was the development of a program logic model, enabling a clear understanding of how service activities translate into outcomes and impact.

Stakeholder engagement was central throughout, ensuring the evaluation reflected operational realities and community perspectives. Thisapproach provided a clear, evidence-based foundation to inform co-design andthe development of a strengthened, community-led model of care.

Outcome

The evaluation provided Nunkuwarrin Yunti with a clear, evidence-based understanding of the performance and strengths of its maternal and child health program, alongside practical insights to inform future development.

HealthQ’s work supported the organisation to:

  • Strengthen visibility of program performance,outcomes, and service delivery
  • Identify key strengths and enablers within the existing model of care
  • Highlight opportunities to enhance integration, flexibility, and alignment to community need
  • Inform a more culturally grounded, community-led approach to service design
  • Establish a strong evidence base to support co-design and future model development

Importantly, the evaluation confirmed a solid foundation on which to build, supporting the transition to a strengthened, First Nations-led Model of Care.
